	Business owners will receive a "Merchant's Information Form"
	Fill out form completely and return to HEZBA.
	Once data is entered into our system, our "Membership Application Form" will be delivered to you by Merchant Coordinator.

	Complete Membership application form and return with Annual Membership Dues in the enclosed envelope	Enjoy the benefits of being a HEZBA Member

	Membership application will be processed.
	An official membership certification along with your membership card will be mailed to you.
