Membership Process…

  • Business owners will receive a “Merchant’s Information Form”
  • Fill out form completely and return to HEZBA.
  • Once data is entered into our system, our “Membership Application Form” will be delivered to you by Merchant Coordinator.
  • Complete Membership application form and return with Annual Membership Dues in the enclosed envelope
  • Membership application will be processed.
  • An official membership certification along with your membership card will be mailed to you.
